The University of Louisville Global Health Center assists refugees, immigrants, and others take the first steps in applying for a Green Card. Having a Green Card (officially known as a Permanent Resident Card (PDF, 1.69 MB) allows you to live and work permanently in the United States.
The Green card process usually requires that you have a medical examination. This exam must be done by a doctor who is authorized by United States Citizenship and Immigration Services (USCIS). Our physicians are certified Civil Surgeons and ensure your medical examination follows all the guidelines outlined by the U.S. Centers for Disease Control and Prevention (CDC).
The Green Card application process requires that you have resided in the United States for 12 months, however, you can schedule an appointment for the medical examination after living in the United States for 11 months.
- Government-issued photo identification, such as a valid passport or driver’s license. If you are 14 years old or younger, bring identification that shows your name, date and place of birth and parent’s full name. Possible forms of identification include your birth certificate (with an English translation) or an affidavit.
- Vaccination or immunization record (such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus, meningococcal, influenza and COVID-19)
- Payment (cash, credit card, debit card, check). We do not bill insurance.
The cost for a Civil Surgeon examination will vary from person to person. However, our program offers competitive pricing and provides the physical examination, required vaccinations, laboratory tests, and a complete I-693 form on-site.
The doctor will complete a comprehensive examination that will include a review of your medical history and a physical examination. The doctor will also test for communicable diseases such as tuberculosis, syphilis, and gonorrhea.
After the exam, the doctor will complete Form I-693 and seal the form in an envelope for you to submit to USCIS. Make sure you get a copy of the completed Form I-693 for your personal records before the doctor seals the envelope. USCIS will not accept the form if the envelope has been opened or altered.
